What do I do next?
There are 3 things you need to do in the first few days after someone dies in hospital:
- Collect a medical certificate of cause of death (MCCD) from the hospital where the patient died.
- Register the death with the Leeds City Council Registrar of Deaths.
- Arrange the funeral - you can use a funeral director or arrange this yourself.
Please contact the bereavement office (10am onwards) for the hospital in which the patient died on the next working day after death. Staff in the bereavement office will be able to discuss with you what happens next and when you may be able to collect the medical certificate of cause of death.

When the MCCD has been completed staff from the Bereavement Services team will contact you and make an appointment for you to collect it from the hospital. Please ensure you call to make an appointment so that we can ensure all the relevant paperwork you need has been completed. They will be able to talk you through the process of registering a death.
