Discharge from hospital

Discharge destination- Chrissy Turner is the discharge co-ordinator for Chapel Allerton Hospital. Chrissy works alongside the rehabilitation team, and will support with your discharge plan from hospital.

Options would include:

• Referral for further rehabilitation - there are several units across Yorkshire.
• Your own home, with or without adaptations and additional care.  Care could include reablement or a package of care.
•  24hr care in either a residential or nursing home.

With any Adult Social Care provision a financial assessment would take place if you require long term support to establish your financial contribution.

Community support-  Our multidisciplinary team may refer you onto the community therapy teams for further support on discharge.  The therapists will discuss this with you prior to discharge.
