Personal Health Record - a new app to help you manage your healthcare and recovery

Personal Health Record (PHR) is a new app which is available to patients under the care of our Fracture Clinic. The app will help you manage your treatment plan and recovery, giving you easy access to all the information you need from your phone, tablet or computer.

How do I access the app?

You’ll be invited to use the app by a member of our team managing your care. They will send you further instructions by text, which will explain the simple steps you need to take to log in.

You'll need your NHS log-in details to sign in, so if you don't have a log-in yet, create one now on the NHS website. This will make sure you’re ready to go when you receive your invite from us.

What will I be able to see?

The Fracture Clinic team will use the app to share information with you. This will include:

  • Video messages from clinicians with specific expertise in your type of injury
  • Instructions on using and fitting any support you might need, such as a sling
  • Phased rehabilitation exercises designed to support your recovery from home

If you have any questions or concerns about your injury at any time, you’ll be able to contact the team by email or telephone.

The team will also use the app to send you secure forms to complete. This will help us track your improvement and monitor how you’re feeling so we can adapt and personalise your care as needed.

We’re rolling out the new PHR gradually across our services and hospitals. In the future, you’ll be able to use more of the app’s features to access information in your hospital health records, track your symptoms, and check test results.

When I’m invited to use the app, who will the text message come from?

The text message will come from Portasana. The PHR app is provided by Wellola, a supplier of digital health solutions across the UK and Ireland, and Portasana is their patient communications platform, which we’ve used to the build the LTHT PHR.

Can I opt out?

Using the PHR app is completely voluntary. If you’d prefer not to use it, we’ll discuss this with you as part of the invitation process so we can help you choose the treatment option you’re most comfortable with.

The PHR app is provided by Wellola, a supplier of digital health solutions across the UK and Ireland. The PHR is built using a product called Portasana, which is a safe and simple patient communication platform.
