Occupational Therapy assessments and treatments focus on an individual patient’s functional abilities (including personal care and domestic skills) this may include provision of equipment, home environmental assessment and risk assessments.
The occupational Therapy Team has a key role within the multi-disciplinary team in enabling timely and safe discharges and working closely with community agencies to resolve housing requirements, supportive care packages and on-going rehabilitation.
Facilitating appropriate discharge is particularly relevant for our ‘Fast Track’ - End of Life referrals which make up one third of the Occupational Therapy caseload in Bexley Wing at St James’s hospital. These often require the management of complex situations to ensure patients are discharged to their chosen destination in a smooth and timely manner.
