Information Relating to Your GP Referral

Following your recent visit to your GP,we would like to share information about the next step in your pathway. If you have received a text or letter we can confirm that we have received a referral letter for you to see a Consultant within the trust.

Your referral has now been forwarded to the consultant for him or her to clinically review.

You do not need to do anything at this moment in time, as we will be in touch with you through either your mobile or landline phone to arrange a suitable date and time for your appointment.

 When we contact you we will be checking all of your personal details such as address, date of birth and GP details.

We will also capture any requirements that you may need for your appointment such as hearing loop or interpreter. If you require hospital transport for your appointment, please can we request that you arrange this with your GP practice.

If any of your personal details change before we contact you or you no longer require an appointment please contact us on the numbers below.

Please contact the Referral and Booking Service if:

-  you have not received a text or letter confirming we have received your GP referral

-  you have not heard from us within 6-8 weeks regarding your appointment
