The Leeds Teaching Hospitals NHS Trust

Bereavement Services

COVID-19 INFORMATION

Due to the current situation, the Trust has needed to make some changes to how we support you when someone dies in one of our hospitals.

Please contact the relevant Bereavement Office using the contact details below, on the next working day after the patient has died. The Bereavement Team will take some details from you and explain what happens next.

You will not need to come into the hospital to collect the medical certificate of cause of death; the Bereavement Office will send the certificate directly to the Registry Office and they will contact you in order to register the death over the phone. The death certificate copy or copies will be posted out to you.

Bereavement Services can arrange to return property and valuables to relatives if they wish. The nominated contact will be phoned to see if they would like the patient's property returned. Instructions will then be given on how to collect the property via a 'drive through'.

Unfortunately, we have taken the difficult decision to suspend visiting in the hospital Chapel of Rest for adult patients. Please note, this only applies to adult patients.

The Bereavement Services team help guide you through the necessary arrangements after someone has died. Please contact us using the numbers below if required.
