Leeds Teaching Hospitals works closely with Carers Leeds to support Carers when the loved one they care for is in hospital. Carers Leeds has Carer Support Workers based in Gledhow Wing at St James’s Hospital, though they cover all sites and you can find more information via: Carer Support Workers.

Dementia Carer Support Worker
Carers Leeds offers a specific service to carers of people with dementia or memory problems in the Leeds Teaching Hospitals NHS Trust. There is a specialist Dementia Carer Support Worker based at St James’s University Hospital who covers all the sites in the Trust. Much of the work focuses on involving carers in discharge planning and ensuring carers are informed about services available to them in the community. The Dementia Carer Support Worker is also able to support, provide advice and signpost carers to other appropriate services where patients do not have dementia or memory problems.

Hospital Liaison Support Worker
Carers Leeds also has a Hospital Liaison Support Worker whose role is to help people, who pay for their own care, find care homes and care packages to support discharge. This role does not just relate to dementia.

Hospital to Home (Support for Carers)
Hospital to home referrals are now going straight to the Advice line: 0113 3804300, where emotional support, as well as information and advice can be given. The referral comes from an Age UK Leeds Hospital to home worker via the online referral page: https://www.carersleeds.org.uk/professionals-referral-form/.
Carers Leeds Advice Line: 0113 3804300 is available to all carers, caring for an individual with a disability or illness that live in Leeds, and is available from Monday -Friday 9am - 5pm. Depending on the illness of the person they care for, we can then offer one to one support with a specialist team such as Dementia, Mental Health and all other conditions will be supported by our outreach or office team depending on the carers ability to leave the cared for any time.
