You have been seen by an Orthotist or Orthotic assistant at the Orthotic department. You have been prescribed and supplied with an orthosis to help with your condition.
An orthosis can be:
- prescription footwear;
- insoles;
- ankle foot orthosis (AFO), ankle supports or braces;
- knee braces;
- spinal supports; and
- collars.
Or any other device / splint or support supplied to you by your Orthotist.
We do not make appointments for you to come back automatically after we supply your orthosis. This is to make sure that we use our appointments in the most efficient way possible.
We have an open review policy, which we describe below.
Open review policy
- You have a ________ open review with the Orthotics service. This means that you can make an appointment to see us within ________ of your last Orthotics appointment, without the need for a new referral.
- Please contact the department to request a review if:
- you have altered in size so the orthosis no longer fits;
- the orthosis is uncomfortable, or rubbing;
- the orthosis has stopped working; and
- the orthosis is broken.
- When you contact us for a review appointment, please give as much information as possible; for example, why you need a review and what problems you are having. This will help us to arrange an appropriate appointment.
- When attending your review appointment, please bring all your prescribed orthoses, even if you no longer use them.
- If we have not seen you within 3 years of your last appointment, please contact your GP for a new referral to the Orthotics department as your condition may have changed.
Repairs
- Please check your Orthoses regularly for signs of wear and damage.
- If repairs are needed to your orthosis, please hand these into the Orthotic department reception with your full name and address and details of the repairs required.
- Please ensure all items handed in are in a clean condition.
- When your orthosis has been repaired, we will send you a letter or a text so that you can come and collect your orthosis.
- If we cannot repair your orthosis, we will contact you and you may require a review appointment so that we can arrange a replacement.
