Making a Subject Access Request
We are required under the Act to respond to Subject Access Requests (SARs) no later than 30 calendar days following receipt. If, having fully considered your request, we require further information in order to respond, we will request this from you as soon as possible. In all such cases, the 30-calendar day clock shall begin only once we have received the information required in relation to the request in full.
In Accordance with ‘Recital 64’ of the UK General Data Protection Regulations (GDPR), the Trust is required to take reasonable measures to verify the identity of the individual who requests access. Owing to this, the Trust is unable to comply with your request until they are satisfied of the identity of the requester and patient (if different).
To make this verification, the Trust requires a complete application along with two copies of the following ID:
- Passport
- Driving licence (physical or digital copy)
- Birth certificate
- Bus pass
- Residency/ID card
- Death certificate
- Power of Attorney (Health and wellbeing)
- Signature of consent – should the SAR form have a formal written signature you are only required to provide one form of ID.
Should the request be made via email, and no formal written signature of consent can be provided, two copies of the above pieces of ID will be required.
Please ensure that only photocopies of identification are submitted and not original documents. The Trust cannot accept responsibility for the handling or return of original personal identification.
