Patients are no longer able to contact our service to refer themselves. All referrals must now be made via the Choose and Book System by the treating clinician.
All patients require an appointment prior to coming to the department, we do not accept drop-in patients. If the patient does not have an appointment on the day, or has already produced their sample off-site, this sample will be rejected.
Instructions to make a referral within the Choose and Book System are as follows (select as below):
- Speciality – Gynaecology
- Clinic Type – Infertility
- Sex Treated – Male
- Organisation or Site name – Seacroft Hospital
Once a referral has been issued, it will be triaged and added to our waiting lists. The patient will then be sent an appointment through our Referral and Booking Service (RBS) at LTHT. The patient will automatically be enrolled into the patient hub and will receive instructions via a text.
If the patient has not opened the patient hub to view or manage their appt within a few days they will be sent a letter and leaflet through the post. The patient hub is the portal within which a patient can cancel or reschedule their appointment and they will be provided with a number for the RBS to contact them if a phone call is required.
For the treating clinician, when making referrals:
- Please stipulate what testing the patient is requiring; an Infertility, Post Vasectomy or Vasectomy Reversal. Please also inform us if this is the first referral, or a repeat test. Please be mindful that we do not accept the Dart referral form. This is solely for Leeds Fertility service and not the Andrology service.
- Please ensure that you continue to give the patient a copy of the request form to bring with them, the top section of the form can be completed by the referring clinician. Please also provide a copy of the appropriate information leaflet, links below.
