
The upcoming pilot of the app, supported by funding from the Leeds Hospitals Charity, gathers patient information on cognitive or psychological challenges from conditions like traumatic brain injury or dementia.
It digitises assessment tools, helping the multidisciplinary care team to access and understand the complex needs of each patient. Melo also aims to digitise the process of recording behavioural data to replace paper-based forms, streamline procedures to improve patient’s movement through their care journey and integrate systems to enable teams to better be able to meet patients’ needs.
The app also has the ability to smooth the process of handover between teams, standardise the information on care cases and enables data to be extracted across a number of devices and platforms to enable clinicians to have a joined-up view of patient care.
This will allow the care team to detect any escalating behaviour early and therefore intervene to scale down any issues and avoid incidents that can be distressing for both the patients and teams involved.
