Every day more than a million people are treated safely in the NHS. Occasionally, things go wrong, or an unexpected event occurs. These are known as patient safety incidents.
A patient safety incident is any unintended or unexpected incident which could have, or did, lead to harm for one or more patients receiving healthcare, and can range in scale from the most minor to the other end of the scale.
Leeds Teaching Hospitals has implemented NHS Patient Safety Incident Response Framework (PSIRF). The Patient Safety Incident Response Framework Policy sets out our approach to developing and maintaining effective systems and processes for responding to patient safety incidents for the purpose of learning and improving patient safety. It embeds patient safety incident response within a wider system of improvement and prompts a significant cultural shift towards systematic patient safety management.

Our Patient Safety Incident Response Plan sets out how we will respond to patient safety events during 2024-2027. Whilst the plan sets out priorities and approach, there may be changes during this period. We will remain flexible, consider the specific circumstances in which patient safety events occurred, how we can respond to improve our services, and focus on the needs of those affected. Our plan sets out the national priorities, defines our local priorities and our approach to reviewing these as well as setting out specialist review processes that are becoming embedded.
