How your care is planned
Your treatment is carefully planned by a group of health professionals who are specialists within the field of haematology. This group is known as the Multi-disciplinary Team (MDT). The MDT meet together regularly to discuss and plan your treatment.
The MDT is made up of the following staff:
- Consultant Haematologist
- Consultant Oncologist
- Specialist Nurse or Key Worker
- Consultant Radiologist
- Consultant Histopathologist
- MDT Co-ordinator
- Research Nurses
The role of the MDT is to provide a high standard of patient care. Following each meeting, information about your diagnosis and treatment options, is documented in your notes and on our computer records. This information is discussed with you at your clinic appointment, at which we encourage you to ask any questions or raise any concerns about your treatment and care.
You will also be offered written information about your diagnosis, treatment plan and copy letters of consultations if you wish.
Key Workers
Your key worker is a health professional who, with your consent and agreement, takes a key role in co-ordinating your care. They provide a point of contact when you need to know where to go for help, advice or information. They act as the link to the teams involved in your care, have an understanding of the care you are receiving and are able to support you with any queries you may have. They will also have another role in your care, for example, they may also be your clinical nurse specialist or district nurse.
