As part of your care one of our clinical team will invite you to take part in two questionnaires; this can help us to understand what symptoms you have and how we might be able to best support you.
For these assessments, we ask you to complete the forms to tell us about any symptoms you may have. Filling this in before your assessment helps us to make sure the conversation we have reflects your own view of your current symptoms.
We have sent you the questionnaires via text message which allows you to complete them on your mobile phone and return it to us. You will receive a text with a link to the form which you can open and complete. On completing the forms, it will automatically be stored securely in your hospital electronic patient record.
If you would rather have a paper copy of the forms to complete that is ok or we can even just ask the questions when we are talking to you if you would prefer not to complete a form.
It is important to note that you shouldn’t use the form to tell us about any worries, concerns or symptoms that require urgent attention, as the returned forms are not being immediately monitored. Please use the normal routes you have (GP/999/111) if you require urgent medical advice.
If you have not completed the questionnaires prior to your appointment, then you can delete the text with the questionnaire link once this has taken place as we will gather the required information whilst talking to you.
If the questionnaires are not appropriate to you, you do not have symptoms, or you would prefer not to complete them please complete the first question to decline assessment.
