Two sample rule definition
Blood Bank must ensure that there are TWO distinct samples from a patient that have generated the same blood group from both samples before ABO matched components are issued.
7 day sample validity:
If a transfusion is required, then a sample needs to be taken within 7 days prior to the transfusion. Exceptions are pregnant women; recently transfused patient and patients 3 months post solid organ transplant where the 72hour sample validity rule will remain. This is to ensure that that the blood components matched by the laboratory staff is as accurate and as safe as possible for our patients.

Sample Validitiy on ICE
The 72-hour sample validity rule remains in place for patients who have had a transfusion within the previous 3 months, those who have undergone solid organ transplants within the previous 3 month and pregnant patients.
All other patients will need a sample taken within 7-days prior to the transfusion. Please review sample collection date and time to check sample validity for your patient.

Sample labelling requirements: Samples must include Full Name, DOB, and Hospital Number. Samples must be signed and dated. Samples can be electronically labelled at the bedside using Blood Track Tx system. To obtain access to blood track Tx you must first complete CHS132 competency.
Handwritten samples are also accepted.
