Potassium is excreted by the kidney into the urine under the control of aldosterone. Urine potassium is measured as part of the urine U&Es panel. Clinical indications for requesting this test include the investigation of hypokalaemia, hyperkalaemia, states of hypoaldosteronism or hyperaldosteronism, and type 4 renal tubular acidosis. A measurement of urine potassium concentration is required to calculate the trans-tubular potassium gradient (TTKG).
