PPM+ is designed to consolidate data from a number of separate systems, allowing clinicians to view a single patient record which includes city-wide information from a number of different organisations. This provides a rich, informed view of our patient’s wider needs.
Leeds Care Record is a great example of this in practice, bringing together important information about patients who have used services provided by their GP, at a local hospital, community healthcare, social services or mental health teams.

Leeds Care Record – General Practice
It has significantly sped up our process of obtaining information from the hospital and eliminated the need to search for patients who have been admitted. It’s also fantastic for patients, as they receive up-to-date information from our GP. For example, we had a patient who was admitted via emergency services. The GP was able to track the patient’s journey, including which part of the hospital they were admitted to. When the patient returned to our care, we were able to receive their hospital letters almost instantly, whereas this would typically take 7 to 10 days.
