Cellular Pathology Change Requests and Service Requests
To request any changes to diagnostic tests or pathways, or to request histopathology service, please complete the Cellular Pathology Change Request Form.
When you open the above form, you will see three request types. Please select the type that best matches your request:
Internal Diagnostic Change Request
For change requests originating within the LTHT Cellular Pathology Department.
External Diagnostic Change Request
For diagnostic change requests originating outside the LTHT Cellular Pathology Department.
External Work Request
For requests for Cellular Pathology services.
Important Information
• All requests must be submitted using the online form.
• The form must be completed in full before submission. Your responses will be saved if you cannot submit the form in one session.
• PDF version of the forms and the request processing flowchart are provided for reference only and to help you gather the required information.
• Requests submitted by any method other than the online form will not be reviewed.
