The service allows the hospital pharmacy team to work with community pharmacists to help our patients get the most out of their medicines.
How could this service help me?
The Discharge Medicine Service creates a direct link with your community pharmacist with the aim to help you achieving the following:
- Improve your knowledge about your medicines
- Understand the changes made to your medicines during your hospital stay
- Discuss any side effects and how best to take your medicines
- Reduce the chance of you being re-admitted to hospital due to medication error
Your community pharmacist also works closely with your GP to support you to feel confident about managing your medicines at home. By understanding your medicines and the best way to take them, you can ensure that they are utilised to help you manage your conditions.
How do I get referred?
The discharge medicine service allows hospital pharmacists and pharmacy technician to refer you directly to your community pharmacy for support with your medications after discharge. The process is outlined below:
- You can choose which pharmacy is best for you
- When you have given your consent, we send an electronic referral which includes a copy of your discharge summary to your chosen pharmacy. This is a report written by the doctor at the end of your stay
- The community pharmacist will then contact you when you are at home to arrange a consultation about your medicines
