Lasting Power of Attorney and other Mental Capacity Act matters
If you are the donee or donor of a Lasting Power of Attorney (LPA) or are a Court Appointed Deputy registered with the Office of the Public Guardian (OPG), we would encourage you to contact our dedicated Mental Capacity Act team, who can ensure the information is recorded appropriately on the relevant patient record.
You can do this in several ways:
- If you have an OPG access code, you can send this, together with the Donor’s last name, direct to the team by email to: [email protected]
- Send a scanned copy of the full document to the team by email to: [email protected]
- If you do not have access to email, you can send a printed copy of the full document via registered post to the team’s address below. Make sure you include your contact details and return address and the team will confirm receipt and arrange to return the document by registered post
- Contact the team to give them the full name, current address and date of birth of the Donor of the LPA (or person subject to Court appointed Deputyship) and we will contact the Office of Public Guardian direct for any information on their database.
You can also contact the team if you have made a written Advance Decision to Refuse Treatment that you wish to be shared on your hospital record.
