Discharge destination
Our discharge co-ordinator for Chapel Allerton Hospital works alongside the rehabilitation team, and will support with your discharge plan from hospital.
Options would include:
- Referral for further rehabilitation – there are several units across Yorkshire
- Your own home, with or without adaptations and additional care. Care could include reablement or a package of care
- 24hr care in either a residential or nursing home
With any Adult Social Care provision a financial assessment would take place if you require long term support to establish your financial contribution.
Community support
Our multidisciplinary team may refer you onto the community therapy teams for further support on discharge. The therapists will discuss this with you prior to discharge.
