Being a Living Donor

If you are on the kidney transplant waiting list the best treatment option for you is to receive a kidney transplant from a living donor. If you have a friend or relative who would like to be considered as a potential donor for you please ask them to contact the transplant team below. You are unable to telephone on their behalf.

Being a living kidney donor

Here is some information about being a live kidney donor. If you wish to be considered as a living kidney donor please contact the Transplant Co-ordinators below at the centre which your friend or relative is registered. Please speak to your friend or relative before you register your interest as a living kidney donor.

Once you have contacted the live donor team, the next stage of the assessment process is for you to complete and return a donor health questionnaire. Here is a link to the questionnaire which can be accessed either from a PC desktop or a mobile/tablet device. PLEASE NOTE: in order to complete the questionnaire electronically you will need to download and save it.

Alternatively you can print off and post to our address (please note if you live in Bradford, York or Hull we will forward your questionnaire to the appropriate team):
