Book your first appointment
Please contact us as soon as you find out you are pregnant. This is so that we can make your first appointment with a midwife. To register you will need to make a My Pregnancy Notes account using the link below.

For urgent advice – MAC
If you have any urgent concerns about your pregnancy or your health after you’ve had your baby, please call the Maternity Assessment Centre (MAC): 0113 3926731.
MAC is open 24 hours a day 7 days a week.
Changing an appointment or scan
You can also change your appointment online using Patient Hub. This only applies to antenatal clinic appointments (e.g. to see a consultant) and fetal assessment appointments (e.g. growth scans). Please use the link in your text message reminder to view your letter and manage your appointment.
Early pregnancy concerns
If you have any early pregnancy concerns before your first midwife appointment, please contact your GP or NHS 111 for advice.

Maternity complaints or concerns
Requesting maternity health records
Patients can request a copy of their own health records. To do this, please complete an application form and provide a copy of identification by email to: [email protected]. On receipt of this information, the Trust is legally allowed 30 calendar days to process the request. Due to an increase in requests, please be aware that there may be a delay in sending your records. Thank you for your patience and understanding.
If you are pregnant and requesting your records to support your on-going care, you do not need to make an Access to Health request. Instead, please ask your current care provider to email [email protected] for access to your full record.
Maternity Records – Transfer of Care
If you are transferring your maternity care to another provider, or you are a clinician requesting current maternity records for a patient who has transferred their care, please email: [email protected]
Please include the patient’s full name, date of birth, NHS number (if known), and contact details of the new care provider. This helps ensure maternity records can be shared efficiently to support ongoing care.

Fundraising and donations
If you would like to fundraise for us or make a donation to support Leeds Maternity services, please visit the Leeds Hospitals Charity website. Thank you for your support.
If you would like to donate knitted hats or any other baby clothes, please telephone our postnatal wards or neonatal unit to check if we are accepting donations and have space to store them.
Alternatively, if you have pre-loved items, please consider donating them to Leeds Baby Bank who provide vital support and urgent baby essentials for families in need.
