My Pregnancy Notes allows you to view your maternity notes securely online using your mobile phone, tablet or computer. You will be able to view your notes from when you register your pregnancy until you are discharged from Maternity care.

Have you recently found out you are pregnant?
Have you recently found out you are pregnant? You can now self-refer to Leeds and Sanity. Sign up today by visiting www.mypregnancynotes.com. Click Register. You can skip this step if you already have a My Pregnancy Notes account, and once registered, select I am a pregnant woman. Click to verify your email address.
Go to your email address and click on the link in the email to verify your account. Choose a password and click Complete pregnancy registration selection. James’s University hospital, or Leeds General Infirmary. When asked if you have already booked with this hospital, select no. You will be asked about data sharing. If you select agree, data will be shared with Leeds Maternity to assist with your care. Please note, if you select I do not agree, your registration form will not be shared with Leeds Maternity.
Finally, fill in as much of the form as you can to help the midwife plan your care. Please add information about your last period, even if this is an estimate, within 14 working days to arrange your first midwifery appointment. We aim for your first midwifery appointment to be when you are around eight weeks pregnant. You will also be able to view helpful leaflets and your maternity notes online. What are maternity notes?
Maternity notes are the place where we write or record information about your medical history and pregnancy. They are medical records for your pregnancy.
When you have an appointment with your midwife or doctor they will write information in your maternity notes. This includes things like what was discussed at the appointment, test results, vaccines you’ve had and your measurements. What can I do using My Pregnancy Notes?
- Record your Personalised Care Plan
- Note down any questions to ask your midwife at your next appointment
- Add notifications, for example changing GPs or moving away from the area
- Access leaflets and information
- Add Your Maternity Exemption Certificate and MAT B1
- Be remotely monitored, if suitable.
If you have already had your first appointment with a midwife you can still register for My Pregnancy Notes. Please note that it may take up to 48 hours for your records to be merged.

Providing third party access
My Pregnancy Notes allows you to share your records with other health care professionals involved in your maternity care.
This can be done under the “grant access to my notes” section under settings.
Please make sure you are only providing this access to health care professionals involved in your care, as this is confidential information about your pregnancy.
Requesting your maternity notes
If you would like to access your maternity health records or birth notes after you have had your baby, please complete an application form and provide a copy of identification by email to: [email protected]. On receipt of this information, the Trust is legally allowed 30 calendar days to process the request. Due to an increase in requests, please be aware that there may be a delay in sending your records. Thank you for your patience and understanding.
Maternity Records – Transfer of Care
If you are transferring your maternity care to another provider, or you are a clinician requesting current maternity records for a patient who has transferred their care, please email: [email protected]
Please include the patient’s full name, date of birth, NHS number (if known), and contact details of the new care provider. This helps ensure maternity records can be shared efficiently to support ongoing care.
